Because sinus mucosal thickening is common in viral URTIs, it is impossible to distinguish bacterial from viral rhinosinusitis on imaging alone. Gwaltney et al. 7 Treatment of acute sinusitis The goals of treating acute sinusitis are to reduce mucosal swelling, restore normal sinus function, relieve symptoms and prevent complications. Commonly prescribed treatments for acute sinusitis include watchful waiting, saline irrigation, decongestants, antihistamines, corticosteroids, mucolytics and antibiotics.

The prevalence of antibiotic resistance to these bacteria is increasing with approximately 25 per cent of Strep. pneumoniae being penicillin resistant. Interestingly, Strep. pneumoniae also has high rates of resistance to other drugs: a third are resistant to trimethoprim/sulfamethoxazole and macrolides, a fifth to doxycycline and a tenth to clindamycin. The AAO-HNS Task Force recommends that when antibiotics are given for the treatment of bacterial sinusitis, the first-line treatment should be amoxicillin.
